This is my first time attending the SA Men's Retreat.
Lodging Amenities
All lodging will be in "river" or "mountain" cabins with bunks for up to eleven men. Restrooms and showers are in a separate building. Linens are not provided. Make sure to bring pillow, sleeping bag, and towel. For more information on camp facilities click here.

Activities

Many activities are available at or around Camp Tadmor. Some activities cost additional money (due at camp on Friday). These activities are on a first come/first served basis and require a commitment when submitting this form. Please check any of the following below if you are committing to this activity.

Paintball $25 (bring extra set of clothes)
Wood Carving $5
Golf at Mallard Creek ($18 not including cart)
Scholarships
I need scholarship assistance of .
I'd like to donate toward the scholarship fund. (Please write a separate check for your donation.)
I'd like to help raise money for scholarships (for me or for others) by assisting with camp cleanup on Sunday afternoon.
Payment
Payment of $100 is due in full no later than May 17. Payment can be made by:
  • placing it in an envelope marked "Men's Retreat" and putting it in the box on the wall to the right of the church reception desk
  • turning it in to the Men's Retreat registration desk on weekends of May 1/2, May 8/9, or May 15/16
  • mailing it to Salem Alliance Church, attn: Men's Retreat, 555 Gaines St. NE, Salem OR 97301
Registration deadline is May 17, 2010.
